Team du Coeur Registration Form. Please fill out form and send in prior to Placement Days.

Things to bring to audition:
• Copy of US Figure Skating membership card
• Copy of birth certificate if new to team
• $40 for Beginner divisions / $50 for Preliminary and Juvenile divisions

Placement Day is closed to spectators for the Preliminary and Juvenile divisions.
